I-693 Form Requirements
+ Basic health questionnaire and physical examination
+ Lab tests
Tuberculosis blood test. Tuberculosis test comes back positive in 3-5% of cases. If your test is positive, you will need to get a chest x-ray.
Syphilis blood test (ages 15 and up)
Gonorrhea urine test (ages 15 and up)
+ Various age-specific immunizations
This process typically takes 4-5 business days.
Applicants under 14 will require a signature by a parent or guardian.
The following is a summary of the age-specific immunization requirements. See here for more details.
All ages: Flu if applying between October and March
18 and under: All childhood immunizations per CDC (Center for Disease Control) recommendations.
19 - 64: Tdap (Tetanus, Diphtheria, and Pertussis), MMR (Measles, Mumps, and Rubella)*, Varicella (Chicken Pox)*
65+: Tdap (Tetanus, Diphtheria, and Pertussis), Varicella (Chicken Pox)*, Pneumonia
*Individuals 19+ can opt to take immunity tests for MMR and Varicella to confirm immunity and avoid these vaccinations.
If you are pregnant, only Tdap and Flu (during Flu season) immunizations are required.
Please provide any immunization records you have available, even if they are from another country or not in English. If they comply to US recommendations, we can use these records to avoid any vaccinations you have already received.
Immunizations can be received at a pharmacy or clinic of your choice. Most insurance plans cover the cost of the immunizations.
+ Immigration medical exam including required labs: $320
+ Immigration medical exam if your insurance covers the required labs: $250
+ Optional immunity test for MMR (Measles/Mumps/Rubella) and Varicella, if uninsured: Add $80
+ Immigration medical exam for uninsured families of 4 or more, including required labs: $300 per person
+ Uninsured returning applicant (have had your I-693 form done with us in the past): $300
+ Refugee/asylee with Medicaid (Apple Health): $200
+ K Visa holder, who needs vaccination verification only: $150
+ Immunizations: Pay to pharmacy or clinic where immunizations are received. This is covered by most insurance plans.
+ Chest X-ray (required only if your TB test is positive): Pay to X-ray clinic. This is covered by most insurance plans.
+ In the case that you require referral to a specialist for further evaluation or treatment: Extra charge incurs.
